You can only extend your visa in Kathmandu or Pokhara. For a 15-day extension, it's at least $45, plus $3 for each extra day.
The extension fee is 3 USD per day (with a minimum extension of 15 days), and a late fine of 5 USD per day applies in case of delay. Multiple re-entry facility is available for an additional fee of 25 USD.

If you're thinking of traveling to India from Nepal, here's the lowdown, simple and clear.
First off, lots of travelers can now get Indian visas online and hop on a flight to New Delhi. But, if you're planning to cross overland into India from Nepal, you need a visa from Nepal itself.
So, head to the India Visa Service Centre at the State Bank of India, just to the right of the Indian embassy in Kathmandu. They only accept applications between 9.30 am and midday, so be an early bird and get there before 9.30 am to be first in line.
You'll need to bring a printed copy of the online visa form from Indian Visa Online, your passport, a copy of the passport info pages, a copy of the Nepalese visa, two passport photos (51mm x 51mm), and the visa fee.
Come back five working days later between 9.30 am and 1 pm with your passport and visa payment receipt. Leave your passport with the embassy, and you can pick it up the next working day between 5 pm and 5.30 pm.
The visa fee for a six-month tourist visa varies based on your nationality.
If you're just passing through with a transit visa (NRs. 2710 for most nationalities), they issue it on the same day. Just remember, it starts from the date of issue and can't be extended.
